About The Role

BaptistCare is seeking a full time Clinical Care Facilitator to join our Home Services Central Coast / Hunter team. Our team delivers high quality care to support our clients’ independence to remain living in their own home. This exciting and autonomous role will provide case management and clinical care and support to our Home Care Package Clients predominantly in the  Hunter region.

What We Are Looking For

The successful applicant will be a Registered Nurse, experience in aged care or home care (community care) services will be highly regarded. You will have a strong customer focus and demonstrated experience or ability in leadership, case management, quality management and team work. A current NSW driver’s license is essential.
