The End-of-Life Pathway is designed to provide extra support so that this choice can be fulfilled safely, comfortably, and with dignity.
The End-of-Life Pathway is part of the government funded Support at Home program. It provides additional home care services for older people who are assessed as having three months or less to live, helping you remain at home and maintain your quality of life.
The Pathway supports older people who:
If you are already receiving Support at Home, your care provider can request a high-priority Support Plan Review. If you are not yet on Support at Home, a medical practitioner, nurse practitioner, or your representative can submit the form to request a high-priority aged care assessment.

Once approved, the End-of-Life Pathway provides $25,000 in home care services over 12 weeks, with the possibility to extend up to 16 weeks if funding remains.
