One of the ways we use the wellness and reablement approach is by providing Occupational Therapy or Physiotherapy assessment and intervention.

BaptistCare’s wellness and reablement approach is embedded in our Well-Living framework: Be well, think well, connect well. This approach is underpinned by our deeply rooted people-first philosophy.

Physiotherapy and Occupational Therapy tailored for you

When you choose Physiotherapy or Occupational Therapy from BaptistCare, you receive compassionate, quality treatment for any existing issues, along with information on prevention strategies to avoid future injury.

Physiotherapy and Occupational Therapy may be funded through your Home Care Package (HCP) or the Commonwealth Home Support Program (CHSP).

Eligible services available from BaptistCare include:

Home safety assessment – making sure your home is safe
Minor and major home modifications e.g. grab rail installation, external ramp installation
The recommendation of aids and equipment to assist you in your independence
Individual falls prevention education
Cognitive assessment and re-training if you are experiencing memory problems
Pressure care management
Wheelchair, seating and scooter assessments
Re-training and task modification to improve independence in your daily life e.g. getting out and about, household tasks and leisure

How are BaptistCare at home services managed and delivered?

Your services will be delivered by a dedicated Care Facilitator and group of Care Workers, supported by a multi-disciplinary team of Registered Nurses, Occupational Therapists, Physiotherapists, Dementia and Palliative care experts, other Allied Health professionals and an experienced local management team. BaptistCare’s size and experience enable us to provide, or easily access, the full range of services and supports that are eligible under a Home Care Package, from simple help around the home to highly complex clinical care needs.
