How we can help with Home Care
For us, it’s all about you. You are introduced to your own dedicated Care Facilitator who gets to know what is really important to you. This is your person. They will work with our friendly, multi-disciplinary care team and our wider network of quality service providers to meet your unique needs, wants and priorities, which may include:
- Help with everyday activities
- Nursing care
- Modifying your home
- Food preparation and meals
- Garden maintenance
- Exercise programs
- Social groups and activities
- Dementia support
- Online activities
- Transport
- Allied health services
- Shopping
Developing a personalised plan and getting to know you – your health and capabilities, your living arrangements and more. Most importantly we want to know what you want to achieve and how we can best work with you to achieve this.
Your services will be delivered by a dedicated Care Facilitator and group of Care Workers, supported by a multi-disciplinary team of Registered Nurses, Occupational Therapists, Physiotherapists, Dementia and Palliative care experts, other Allied Health professionals and an experienced local management team.
BaptistCare’s size and experience enable us to provide, or easily access, the full range of services and supports that are eligible under a Home Care Package, from simple help around the home to highly complex clinical care needs.
