Depending on your care needs, you can expect some of the following services:
Nursing funded by the Commonwealth Home Support Program (CHSP)
If you are eligible for nursing services through the Commonwealth Home Support Program , you might receive:
- Short-term or intermittent treatment of any injuries or illnesses you are managing
- Ongoing monitoring of any medically diagnosed clinical conditions you are living with
- Injury prevention strategies and education around living well at home
- Treatments and care that promote your independence at home and in the community.
Nursing funded by the Home Care packages program (HCP)
If you are eligible for nursing services through the Home Care Packages program, you might receive:
- Mid to long-term treatment of any injuries or illnesses you are managing
- Ongoing monitoring of any medically diagnosed clinical conditions you are living with
- Injury prevention strategies and education around living well at home
- Ongoing clinical support in meeting complex care needs, such as dementia management or multiple chronic conditions.

Nursing services are available through Support at Home (SaH) or the Commonwealth Home Support Program (CHSP).
