The Restorative Care Pathway is a personalised program designed to help you achieve your goals and regain independence. It includes an initial assessment and Goal Plan, tailored therapy and exercise, support with daily activities, and one-to-one sessions with nurses and allied health professionals. Your progress is regularly reviewed through case conferences, with ongoing support such as equipment, home setup, community connections, and strategies to maintain independence long term..

You can be referred for a Restorative Care eligibility assessment through My Aged Care by calling 1800 200 422 or visit www.myagedcare.gov.au.
Once your referral is submitted, an Aged Care Assessor will contact you to arrange an assessment and determine whether the Restorative Care Pathway is suitable for your needs.
