With BaptistCare supporting you, you can feel confident knowing your recovery is guided by compassionate, professional care.
Our Transition Care service is designed to support you as you recover after a hospital stay, helping you rebuild strength and confidence so you can return home and resume everyday life.
We know recovery doesn’t happen overnight, and the right support can make a meaningful difference at every step.
If you or a loved one are coming home from hospital, we’re here to make the transition smoother.
For more than 80 years, BaptistCare has supported Australians with care grounded in compassion, respect, and genuine understanding.

Your care is coordinated by a dedicated Care Facilitator and delivered by a trusted team of Care Workers, backed by a multidisciplinary network of Registered Nurses, Occupational Therapists, Physiotherapists, Dementia and Palliative Care specialists, allied health professionals, and experienced local leaders.
