Case Management is a conversation between you and a qualified case worker aimed at supporting your wellbeing through setting goals, resolving issues and working together to achieve outcomes that are important to you.

Our Case Worker uses a person-centred approach, which means we view you as the expert on yourself and your experiences; we support your choices and believe in your abilities.

Our approach to outreach means we can come to you and come around you during times where you might need some extra support.

This includes meeting with clients to assist them during appointments and dropping off food support.

We offer crisis food hampers to individuals and families who may be unable to access food, or are lacking the basic items to make a meal.

Our chaplains and volunteers come alongside of people, offering support and a listening ear as they go through life’s challenges and experiences.

It doesn't matter what your background or beliefs are, the spiritual and emotional support we offer is for everyone.

Our chaplains and volunteers are a compassionate presence at HopeStreet, and are there to get to know people, their stories, hopes and dreams. We help you engage in what gives you purpose and meaning in life.
