End-of-life care coordination services are designed to support people with a life-limiting illness to enhance their wellbeing. The care coordinator works with the person and their carers to help them receive the support they need during this stage of life.
The Final Footprints Support Program is available to anyone over 55 with non-malignant life-limiting illness, living in the Newcastle, Lake Macquarie and the Central Coast.

This program is fully funded by the Hunter New England and Central Coast Primary Health Network and there is no cost to access it.
