**Available to residents living in Newcastle suburbs only**
End-of-life care coordination is a service designed to support people with a life-limiting illness to enhance their wellbeing. The care coordinator works with the person and their carers to assist them to receive the support they require during this stage of life.
The End-of-Life Care Coordination program is available to anyone over 65 (or 55 for First Nations people), with a non-malignant life limiting illness, living in the Newcastle Local Government Area.
