What is The Stronger Carers program?

The Stronger Carers: Supporting and empowering carers, is a program designed to assist those carers of people living at home with dementia.

The care coach works with the carer to assist them in building their knowledge, skills and capability to support their role as a carer for someone living with dementia.

This program is fully funded by the Australian Department of Health and Aged Care and there is no charge to attend.

The Stronger Carers is an 8-week coaching program, with 6 weeks in-home visits and 2 weeks via virtual/phone.

This program is available to residents living in Canberra and surrounds

Program details

Coaching is delivered in the familiarity of the home, with a coach providing advice and support to meet individualised care needs including:

Facilitating meaningful local respite options such as in-home, day centre programs, cottage, residential or emergency respite.

Arranging a dedicated Care Worker (CHSP or SaH funded) to assist with transition to respite where needed.

Offering strategies for the carer’s wellbeing and restoration care.

Providing advice and solutions that optimise benefits for care recipients, including physical, emotional, intellectual, social and spiritual wellbeing

Providing education and resources on cognitive and physical reablement activities, as well as compensatory strategies for carers to implement with the care recipient.

Facilitating connections with other carers and organisations for continued support.

Facilitating access to dementia appropriate equipment and technologies.

Providing access to pertinent information concerning the specific dementia diagnosis, disease progression and management strategies

How do I access the program?

The Stronger Carers program is available to any carer of an individual living with dementia at home and located in the ACT and surrounds.

The program can be accessed via self-referral after a screening process is conducted over the phone.
