The Stronger Carers: Supporting and empowering carers, is a program designed to assist those carers of people living at home with dementia.
The care coach works with the carer to assist them in building their knowledge, skills and capability to support their role as a carer for someone living with dementia.
This program is fully funded by the Australian Department of Health and Aged Care and there is no charge to attend.
The Stronger Carers is an 8-week coaching program, with 6 weeks in-home visits and 2 weeks via virtual/phone.
Coaching is delivered in the familiarity of the home, with a coach providing advice and support to meet individualised care needs including:
The Stronger Carers program is available to any carer of an individual living with dementia at home and located in the ACT.
The program can be accessed via self-referral after a screening process is conducted over the phone.
