How do I access the program?

Available to residents living in NSW across Newcastle, Lake Macquarie and the Central Coast.

End-of-life care coordination services are designed to support people with a life-limiting illness to enhance their wellbeing. The care coordinator works with the person and their carers to help them receive the support they need during this stage of life.

Services provided may include:

Assistance in defining and achieving personal goals

Information about specialised services and support available in the community and how to access these services

Coordination of care between services

Education about palliative care and symptom management

Connection with services in the community to support client to develop advance care plans and directives, and appoint legal representatives

Referrals to service that assist with everyday activities

Assistance when moving between care settings

Support to access culturally appropriate services and resources

How do I access end-of-life care coordination services?

The Final Footprints Support Program is available to anyone over 55 with non-malignant life-limiting illness, living in the Newcastle, Lake Macquarie and the Central Coast.

Who can refer:

• The person with the life-limiting illness
• Family / friends / carers
• GP’s, nursing and allied health professionals
• Local palliative care services 

This program is fully funded by the Hunter New England and Central Coast Primary Health Network and there is no cost to access it.
