End-of-life care coordination is a service designed to support people with a life-limiting illness to enhance their wellbeing. The care coordinator works with the person and their carers to assist them to receive the support they require during this stage of life.
The Final Footprints Support Program is available to anyone over 55 with non-malignant life-limiting illness, living in the Newcastle or Lake Macquarie Local Government Area.

This program is fully funded by the Hunter New England and Central Coast Primary Health Network and there is no cost to access it.
