Nursing care that makes a difference
When you receive clinical nursing care from BaptistCare at home, you can expect caring, quality treatment for any illnesses or injuries you might be living with, as well as support with education and prevention strategies for further injury.
Nursing care can be funded through the Commonwealth Home Support Program (CHSP) or the Support at Home program (SaH).
Depending on your care needs, you can expect some of the following services:
Nursing funded by the Commonwealth Home Support Program (CHSP)
If you are eligible for nursing services through the Commonwealth Home Support Program, you might receive:
- Short-term or intermittent treatment of any injuries or illnesses you are managing
- Ongoing monitoring of any medically diagnosed clinical conditions you are living with
- Injury prevention strategies and education around living well at home
- Treatments and care that promote your independence at home and in the community.
Nursing funded by the Support at Home program (SaH)
If you are eligible for nursing services through the Support at Home program, you might receive:
- Mid to long-term treatment of any injuries or illnesses you are managing
- Ongoing monitoring of any medically diagnosed clinical conditions you are living with
- Injury prevention strategies and education around living well at home
- Ongoing clinical support in meeting complex care needs, such as dementia management or multiple chronic conditions.
