Delivering continuity of care with our MyTeam®approach

Putting people first means listening, and learning what matters to our residents. For many, this is having the same familiar faces provide personal care each day.

Our “MyTeam®” approach brings together residents, their families, and care workers in a partnership that ensures each person receives care from the same group of team members where possible. It is this continuity of care that enables deeper connections, a better understanding of residents’ needs and preferences, and support for a meaningful life.
